If the type of care you need is best delivered in a care home, then you will first be assessed to see whether you require residential or nursing care. Once you have chosen your care home, the Home Manager will complete a pre-admission assessment to ensure that the home is able to meet your individual and specific needs – this is a requirement under current legislation.

Residential Care

Residential care homes provide what is referred to as “personal care”, that is, help with washing/bathing and dressing and giving medication if appropriate and required. If you come to live in an OSJCT care home, you will have your own room, sometimes with en suite bathroom. Our professionally trained care staff are available 24 hours a day to help you; we have housekeepers to undertake domestic tasks and our cooks and kitchen staff provide full catering services. All our homes have Activity Co-ordinators who work with residents to provide a wide range of different activities. It is also possible for residents to continue with membership of outside clubs and organisations if they so wish. To find out more about our residential care, take a look at our individual homes or contact us.

Nursing Care

A care home which provides nursing care may be more appropriate if you have an illness or a disability which means you need a higher level of care. Such a care home is required to have a Registered Nurse on duty 24 hours a day. OSJCT nurses – as is the case for all our care staff – have on-going training and development to ensure their clinical skills are kept up to date.

As with residential care, care staff will consult with you to complete your own personal care plan, which will guide staff with regard to your individual needs and preferences. These plans are regularly reviewed, evaluated and updated, always in consultation with you.

Dementia Care

We know that making a decision about moving into a care home is never easy. This is especially true for people with dementia, and their families or carers.

OSJCT homes offering dementia care are led by Managers with a special interest and expertise in this area. They are knowledgeable and understanding of the particular difficulties and anxieties faced by people with dementia and their families. We provide specialist, dementia specific training for all our staff, from carers to housekeepers, cooks to handymen. This training has been designed in conjunction with and is accredited by the Alzheimer’s Society. We strive to deliver person-centred care, focusing on the person as an individual, using care plans and life history work to ensure that our service is tailor made to meet each person’s needs.

Intermediate Care

Intermediate care is designed to assist recovery from illness or a stay in hospital, or to avoid hospital admission. The aim of this service is to support individuals to return to their own homes and to maximise independent living. Respite Care

Caring for someone is a big commitment and can be hard work. It’s very easy, when you are focusing on looking after someone else, to forget about looking after yourself. Respite care can give you the break you need with the reassurance of knowing that the person you care for is in a safe environment, with 24 hour care available, being looked after by people who will respect them and treat them with dignity.

Equally, if you are the person who needs care, taking a break from your usual routine can make a real difference, giving you an opportunity to meet new people in a different, but safe and homely environment.

Respite care also enables you to “get to know” a care home, its staff and the other residents. This could be a real benefit in the future, should your care needs change and a more permanent move to a care home become necessary.

Day Care

The main aims of Day Care Centres are to give carers a break and also to provide an informal base for social interaction and company to offset isolation and loneliness. Day Care services are often provided in a semi-structured manner between 9am and 4pm, Monday to Friday. However, Day Care Centres are adapting to a growing demand, and services are increasingly being made available seven days a week, 52 weeks a year, and for longer periods during the day.

Within Day Care Centres, as well as companionship and theraputic activity, you will usually find a range of additional services that may be chosen, such as, for example, laundry, bathing, take away meals, hairdressing and foot care. Day Care is also becoming established as a specialist service with Day Care Centres offering expertise in such areas as dementia and re-ablement.

Day Care is also being seen as an option that may be taken up as and when needed, rather than a longer term commitment. Clients’ attendance may vary from week to week or month to month. They may attend for mornings or afternoons only and it may also be added to short periods of respite care to enable carers to have free time around particular events or personal need.

Extra Care

There are an increasing number of extra care schemes being built around the country. Residents live in their own apartment – typically one or two bedroomed – but are provided with care support packages, tailored to their individual needs. This means that residents maintain a high degree of independence but have the reassurance that care and assistance is available as they require it.
