Intermediate care is used primarily as a 'bridge' between home and hospital.

Intermediate care is what we call the transition period between home and, most usually, a hospital stay.

It can be needed when someone leaves hospital, to help them recuperate and assist in rehabilitation before a return home, or it can, sometimes, be used to avoid an admission to hospital.

The purpose of intermediate care is to support individuals to return to their own home and to maximise independent living.
