Grace Care Centre First to Join New Healthcare Programme
Grace Care Centre is the first care home in Gloucestershire to sign up for the Connecting Care programme.
Grace Care Centre is very proud to be the first care home in Gloucestershire to sign up for the innovative health and social care programme, named Connecting Care.
Connecting Care is an integrated digital system of medical records and interactions between professionals. Healthcare professionals involved in an individual’s care have easy access to accurate and consistent information, with the goal of improving outcomes for patients and residents.
The system can help to speed up the treatment of an individual by streamlining the workload of health and social care professionals and reducing duplication of information and test results.
Connecting Care users include: GP practices, hospital departments, community teams, mental health trusts, out of hours services and local authorities.
Peter Moore, General Manager at OSJCT Grace Care Centre, said: “We are very proud to be the first care home in Gloucestershire sign up to Connecting Care. We have already seen many examples of where this service has benefited our residents.”
South Gloucestershire Council’s Cabinet Member for Adults and Public Health Cllr Ben Stokes said: “It’s great to see the first care home in our area sign up to Connecting Care. Connecting Care is a secure digital care record system for sharing information which enables professionals involved in your care to have instant, secure access to health and social care records. This ensures they have the latest up-to-date information and can help speed up treatment.”
