A warm welcome to our home

The home offers compassionate, individualised residential, dementia, respite and day care for up to 64 residents. The purpose-built home environment has been specifically designed to support the needs of older people including those with dementia, from aiding memory in day-to-day living and reminiscence therapy, to reinforcing personal identity and navigating around the home.

Cosy lounges are available to relax in, as are safe terraces to take advantage of on sunny days and social activities take place daily for your enjoyment. If you’d like a little more privacy, then your own spacious bed-sitting room, with en-suite ‘wet room’ shower facilities, offers a peaceful oasis.

If you are joining us for day care you will be able to enjoy all of the facilities which include; your own en-suite bedroom, our delicious home-cooked meals, daily activities and use of our jacuzzi bathrooms.

Our friendly, professionally trained team are on hand around the clock to support our residents, ensuring they spend their day how they wish and enabling them to enjoy a fulfilling and independent lifestyle.

The attentive care team focus on the individual needs of the residents, with a tailored care plan for each person. Their aim is to improve the quality of life for our residents by creating lovely, comfortable, homely surroundings and by providing care, activities and other services designed around each resident’s needs and wishes.

 

Meet the team

A talented chef and kitchen team prepare tasty, home-cooked meals. The freshly prepared, delicious dishes are carefully balanced to get the best nutritional value and cater for individual dietary requirements and preferences. Seasonal produce is chosen whenever possible and of course, relatives and friends are always welcome to join.

The menus change daily, and the kettle is always on, so hot drinks are available throughout the day.

Each of the four households have their own personal kitchen-dining room where residents u can take their meals if they wish. Residents are also encouraged to help prepare their own food, and to make snacks.

Whether it’s wet or dry outside, we at OSJCT feel that activities are essential in providing the best caring environment to live in.

Hobbies don’t simply stop when living with us at The Lakes Care Centre. We have a dedicated Activities Co-ordinator who works closely with our residents to create a programme of fulfilling activities and events, tailored around each resident’s requests, hobbies and interests. The programme of events and activities provide a source of fun and laughter, to help each resident live their life to the full and make new friendships.

We are also fortunate to have the support of many volunteers of all ages from the local community, strengthening our links to this thriving market town. Here is a sample of activities on offer:

Many of the activities feature Reminiscence Therapy to aid those living with Dementia to remember events, people and places using sight, touch, taste, smell and sound.

  • Friday Coffee Morning: 10.30am – 12pm
  • Knit2gether: every Thursday 2:00pm - 3:30pm. A group led yarn-based activity for all abilities.

Meet the team

Home Manager

Mandi started working in care when she was 18 in a small dual registered nursing home, before going on to becoming a senior carer and mentor. It was very evident from the beginning that Mandi had the knowledge, skills and experience to lead a team and continue her mentoring to help develop new starters with the high standards she was taught right from the beginning. (She forgets how many times her beds had been stripped by the Matron, as they were back then!)

After a couple of years she was offered a managerial role, leading a team over two residential units and after six years as residential care manager, Mandi stepped into the home manager role to lead the home. After nine years at the helm, she was after a new challenge and managed an 80 bedded nursing home. It was here Mandi, with her team, secured a good CQC inspection for the first time in eight years. She also gained an accreditation for the dementia care delivered, and then the home went on to win a National South West award for its dementia care.

Mandi has recently joined the Trust, and with the home's first 'Good' CQC rating, she has high aspirations for the home's future and hopes to secure many awards with her team.

Activity Coordinator

Beky is one of two Activities Co-ordinators here at The Lakes. Having recently joined the Trust, Beky is focused not only on meaningful activities and general well being within the home, but also on setting up links within our local community and encouraging a younger demographic to come through the doors to bridge the gap between generations.

Beky hasn't always worked in care. Having started out in the marketing and design sector, Beky decided to move to a role that was people-centred and is devoted to promoting engagement and independence amongst the residents. In her spare time, Beky enjoys volunteering within the local community, working alongside those with mental health and learning disabilities. With this wealth of experience, she is hoping to gain further qualifications in the Health and Social Care sector.

Bursar

Alison Chapman is the Bursar at The Lakes, but she hasn’t always worked in care. Five years ago, Alison moved from the business sector to care after longing to make a difference in the lives of others. Alison’s administration and customer service management skills that she had acquired over 25 years enables her to support all those working alongside her in the home.

Alison has studied Dementia Care level 2 whilst managing a volunteering team at a Hospice, and has recently gained level 3 in Dementia Care.

Her favourite part of the job is seeing the residents happy, entertained and being well looked after. She says she feels a great achievement in knowing she has contributed to improving the lives of others.

Deputy Manager

Jessica began her role working with the Trust in 2008 and found her passion for supporting people to live well and advocate for the best care possible. Throughout the years of working in care homes, Jessica gained valuable experience and knowledge; She started as a Carer before moving into an in-house trainer role and then on to be a Care Leader. Through natural progression, Jessica found a keen interest in educating others, and in 2012 decided to leave her role as a Care Leader and support best practice care by training others, becoming a Health and Social Care Assessor. In her role as a QCF Assessor, Jessica secured the skills to later form part of the Trust’s Learning Development Team. However, it was during this period that Jessica realised her real passion was back in the home with the residents and accepted the Head of Dementia Care role at The Lakes. Since joining the team she hasn’t looked back since. In 2019 Jess accepted the position of Deputy Manager and she is a huge asset to the team.

Jessica has a firm understanding of how to best provide a safe, effective and personal experience for the residents who are living with dementia. Jessica feels privileged to be part of the team at The Lakes and looks forward to a happy future for the home.
